Unit 40 Athletics

The Board of Education determines that participation in co-curricular activities is a privilege. The student participant is expected to show good sportsmanship at all times. If a student participant chooses not to comply with the rules and requirements set forth below, the privilege to participate in activity may be lost. The following rules will go into effect when the student signs the Student Handbook or the Activity Code and will continue to apply until the end of the participant's career at the school in which the code was signed.

Final Forms is the website we use to track all student medical forms, permission forms, and academic eligibility in order to ensure that your student is eligible for sports. For parents, it is the online form management system that collects critical information for each athlete. Parents prefer this information collection method because it eliminates repetitive paperwork by saving and pre-populating their child's data year-to-year. Parents only need to register once, allowing them to verify, update, and digitally sign required documents from any device.
