Athletic Registration via Final Forms is now open for the 2026-27 school year!
All students' grade levels and school assignments have been updated to their 2026-27 status in Final Forms. Any student planning to participate in Unit 40 athletics or activities must complete a 2026-2027 registration through Final Forms.
Parents: Please log in to Final Forms to review and complete your student's registration:
Getting Started
- Select all sports and activities you know your student plans to participate in
- If you're unsure, you can add activities later
- If plans change, activities can also be removed at any time
- Review all forms and information
- Information entered last year has been saved
- RETURNING ATHLETES - Verify that all information is current and make updates as needed
- NEW ATHLETES - Fill out requested information
- Complete the annual forms by electronically signing each one for the 2026-27 school year
Athletic Physicals
Physicals may be uploaded directly into Final Forms by scanning or photographing the completed forms. Please ensure all uploaded physicals include:
- Date of examination
- Physician's signature
- Parent/guardian signature
- Student signature
Because office hours are limited during the summer, mailing or uploading documents is recommended until school resumes. If you prefer to submit a paper copy, please mail it to:
- Effingham High School (Grades 10-12)
- Effingham Junior High School (Grades 5, 7, and 8)
Important: Incoming 6th and 9th grade students must submit their required school physicals directly to their school office.
High School Students
High school students are also required to review and sign forms. Once a parent has completed all required forms, an email will be sent to the student with instructions for completing their portion of the registration.
